Compassion and Excellence with Integrity and Grace.

Jose Mendoza, MD

Medical Director

Bridgeway Healthcare and Hospice  understands the special relationships that exist between most patients and their physicians. Dr. Mendoza's role as Bridgeway’s Medical Director is to partner with the patient’s physician in an attempt to provide collaborative end-of-life care. The Bridgeway team welcomes involvement from those existing physician relationships and strives to maintain communication necessary to manage that care.

As a member of the medical staff, the goal is to provide successful symptom management and improve quality of life by guiding and supporting you through the end of life experience.

The Bridgeway team is dedicated to empowering patients and their families. If you or a loved one is facing a life-limiting illness, you may seek assistance and support through hospice services by calling the friendly staff at Bridgeway Hospice.

Erin Viator-Jolet, RN

Assistant Director of Nursing

Lafayette, LA

A 2007 graduate from LSU – Eunice with a degree in nursing. With 11 years of nursing experience and having worked with dialysis patients for over many years and ICU as well, making the transition to hospice and palliative care was an easy decision. In early 2015, Bridgeway Hospice presented an awesome new career opportunity and challenge to me that has been one of the best decisions I have ever made. The passion and understanding shared with the employees, patients, and families is unwavering. “I love Bridgeway Hospice & our entire team. We truly do strive to provide the best care for our patients & their families. There’s nowhere else I’d rather work”. When not at work, spending time at home in Youngsville with my husband and 4 dogs is my pastime.

Heather Mayard, RN

Assistant Director of Nursing

Lafayette, LA

I have been an Registered Nurse for over 10 years, graduating from McNeese State University. Starting in healthcare as a CNA, then an LPN, and now as an RN has helped me to see how we all play an integral role in caring for patients. Working at an LTAC facility as charge nurse has equipped me with solid management skills and abilities. My previous experience in Home Health helped prepare me for this hospice experience. I can now say that Hospice is really the reason I became a Registered Nurse; I have found my calling in hospice and at Bridgeway Hospice of Acadiana. It is truly a privilege to care for the patients and families at their end of life experience and at their most critical time of need. When not working, I enjoy spending time with my husband Ryan and 6 children.

 Hollie Carlson, RN

Director of Nursing

Lafayette, LA

I am a graduate of UL Lafayette, and I have over 9 years of nursing experience. Spending the majority of my nursing career in ICU at a local acute care hospital, I also have experience in management at a local Hyperbaric facility. Hospice is my new career passion as I have held many roles in my two-year hospice tenure with Bridgeway Hospice of Acadiana; RN field case manager for one year and Assistant Director of Nursing for the past year. Joining the Bridgeway of Acadiana team in 2016, my passion for serving people, especially the hospice patients and families has been one of my greatest rewards. When not at work, spending time with my family (husband Stuart and 2 children, Evan and Aubrie) is of utmost importance.

Our services and passion for caring extend to all regardless of diagnosis,

gender, national origin, race, color, creed, sexual orientation, disability, age or place of residence.
